Fall Registration Form 2011-2012

Registration process includes 3 steps:
1.  Fill out and submit this registration form.
2.  Read Parent Information Guide Page on this site and fill out and submit bottom portion.
3.  Print and fill out release form from this site and bring it on your child's first class day.
All 3 forms are necessary for students to participate at our studio.

Monday 5:15 - 6:15 Ballet III (ages 10+) Studio B
Monday 6:15 - 6:45 Pointe I (teacher placement only and must also take Ballet) Studio B
Monday 6:45 - 7:45 Ballet IV (teacher placement only) Studio B
Monday 7:45 Pointe II (teacher placement only and must also take Ballet) Studio B
Monday 7:45 - 8:45 Ballroom (ages 14 to adult) Studio A
Tuesday 3:45 Jazz I (5-6 year olds) Studio A
Tuesday 4:30 Cheer Bears (4-6 year olds) Studio A (Cheerleaders must also enroll in tumble.)
Tuesday 5:15 - 6:15 Tumble I (ages 4-6) Studio B
Tuesday 5:15- 6:15 Cheer II (ages 7+) Studio A (cheerleaders must also enroll in Tumble.)
Tuesday 6:15 Tumble II (ages 7+) Studio B
Tuesday 7:00 Jazz II (ages 7+) Studio A
Tuesday 6:15 Jazz III (ages 10+) Studio A
Tuesday 7:00 Theater (Sing, Dance, Act) (ages 10+) Studio B
Thursday 9:00 A.M. Tumble Bears (3-4 year olds) Studio A
Thursday 9:45 A.M. Ballet (3-4 year olds) Studio A
Thursday 9:45 A.M. Tap I (5-6 year olds) Studio B
Thursday 10:30 A.M. Ballet I (5-6 year olds) Studio A
Thursday 10:30 A.M. Tap II (ages 7+) Studio B
Thursday 11:15 A.M. Ballet II (ages 7+) Studio A
Thursday 12:00- 1:00 Ballet III & Pre-Pointe (ages 10+ or by teacher placement) Studio A
Thursday 4:30 Lyrical I (ages 10+) Studio A
Thursday 5:15 Hip-Hop II (ages 10+) Studio A
Thursday 6:00 Lyrical II (teacher placement only) Studio A
Thursday 6:00 Ballet I (ages 5-6)
Thursday 6:45 Hip-Hop III (teacher placement only) Studio A
Thursday 6:45 - 7:45 Ballet II (ages 7+) Studio B
Thursday 7:30 Jazz IV (teacher placement only) Studio A
Thursday 7:45 Adult Beginner Ballet Studio B
Thursday 8:15 Drill Team Prep (ages 12+) Studio A

All classes are 45 minutes unless otherwise noted.
After submitting form please go to "pay registration fees" page.
Then please read Parent Information Guide.  Click the submit button when you are done.  Thank you!
